There are many changes happening in health care and we want you to benefit and participate along with us.
We are happy to announce that our practice is in the process of pursuing recognition by the National Committee on Quality Assurance in Washington, DC as a Patient Centered Medical Home. What does this mean to you?
A Patient Centered Medical Home is a model of delivering health care that incorporates a patient-doctor partnership to improve the outcome of medical care for you. We will ask you to be a willing partner in our medical “home” as we strive to improve communication, provide care in a culturally sensitive manner, expand our availability and be the first place you call when you have any healthcare concerns.
Over the coming months, our practice will be asking for your participation in our partnership which comes with responsibilities to maintain updated contact information, obtain preventive screenings when advised, closely monitor chronic disease and communicate with us about all aspects of your medical care.
The Patient Centered Medical Home concept has a strong basis in preventive medicine. This means as our partner you will be expected to keep up to date with things like your regular examinations, screenings for chronic diseases and immunizations. You are asked to communicate with us about all aspects of your medical care.
Our staff and Dr. Peterman will listen to you, ask you to give feedback and respond to your needs in a timely and sensitive manner. These innovations may come with growing pains and we ask your support and patience as we transition and fully implement our Patient Centered Medical Home.
