The Patient Portal is Now Up & Running!

We now have a patient portal available for our patients to use. This will allow you to have real-time access to your medications, labs and upcoming appointments. You will also be able to request appointments, request refills, and message a member of our office through email.

Signing up is Simple.

  1. Get your personalized activation code
  2. Log on to the Portal through our website
  3. Be able to view your chart!

You will need to aquire your personalized activation code either at your next appointment or by calling our office and your code can be given verbally.

Once you have Aquired your Activation Code

To get started using the portal, click on the link for the patient portal located at the top right corner of the screen. Click on the green video icon to view a tutorial on how to activate your account. Once you are ready, click on the link activate patient portal account.

You will then be prompted to fill out some basic personal information, as well as create a login name and password. Be sure to enter your name and date of birth correctly, including any suffixes. Please be sure you are entering your LAST NAME first.

**Your activation code is case sensitive and will need to be entered exactly as you are given. Make sure your Caps Lock is not on!

The patient portal is to be used for NON-URGENT issues only.

Please be sure to be as thorough as posssible when filling out your personal information and medical history. This will save us time on the day of your appointment, and help us provide you with the best medical care possible.
